How to add custom fields to your contact form

Add custom fields to your contact form from the Contact form section in the Shopify theme editor. You can add text, text area, phone, checkbox, and select dropdown fields, mark text fields as required, and choose which options appear in a dropdown.
Field types and settings
Each block you add is one field. Text, checkbox, and select dropdown blocks have no limit; text area and phone blocks are limited to one per form.

Text area and phone fields are always required for customers, and you can only add one of each. The default contact page already includes one phone and one text area field.
Add a field
Add and configure each field from the Contact form section in the theme editor.
In the theme editor, open the page that uses the Contact form section.
Select the Contact form section, then choose Add block and select a field type.
Enter the field's Label.
For Text fields, turn on Required if customers must complete it.
For Select dropdown, enter the choices customers can pick in Option 1 through Option 10. Blank options are omitted from the dropdown.
Reorder and save
Fields render on the form in the order you set in the editor.
Drag the field blocks into the order you want.
Save the theme.
Test the form
Verify your fields appear and submit correctly on the storefront.
Open the contact page on your storefront.
Fill in your custom fields and submit a test message.
Confirm the submission arrives at the email address on your store's contact information.
